Prior Authorization Criteria
Leon Health requires you or your physician, to get prior authorization for certain drugs. This means that you will need to get approval from Leon Health, Inc. before you fill your prescriptions. To find out which drugs need prior authorization, view the Part D Prior Authorization List:

Step Therapy Criteria
In some cases, Leon Health requires you to first try certain drugs to treat your medical condition before we will cover another drug for that condition. To find out which drugs are subject to Step Therapy, refer to the Part D Step Therapy List:
