Credentialing for Physicians and other Healthcare Professionals

Thank you for your interest in joining the Leon Health Plan (LHP) network as a provider. In order to begin processing your request for participation we will need you or your organization to submit information that is required of all licensed physicians and other healthcare professionals.

As part of our credentialing process, LHP uses the Council for Affordable Quality Healthcare (CAQH) to collect the credentialing application data and verify required elements of the credentialing application and process.

Physicians and other healthcare professionals obtain their CAQH provider ID directly fromCAQH by going to the CAQH website and completing a registration form. Once the form is submitted, the applicant will receive an email from CAQH with a CAQH provider ID. Then the applicant will be able to sign in to CAQH ProView and create a username and password, which are used to complete the online application.

If the application information already exists on the CAQH website, please ensure that all data is up to date and you have attested within the last 120 days.
