Welcome to Leon Health Credentialing

At Leon Health, we are committed to maintaining a high-quality network of healthcare professionals and facilities dedicated to delivering exceptional care to our members.

Credentialing is the process of evaluating and verifying a provider’s qualifications to ensure compliance with regulatory, accreditation, and organizational standards. This review may include education, training, licensure, certifications, professional experience, professional liability coverage, accreditation status, and other qualifications necessary for participation in our network.

Credentialing and recredentialing help ensure that participating providers and facilities meet the standards required to deliver quality care to Leon Health members.

Who Can Be Credentialed?

Credentialing is available only to Medical Providers, Hospital and Facilities, Ancillary and Centers, Dental, Vision, Behavioral Health and other that have an active participation agreement with Leon Health.

If you are not currently contracted and are interested in joining our network, please visit our Join Our Network page and complete the Provider Interest Form. Once the contracting process has been completed, you may begin the credentialing process.

How to Request a Credentialing Application

Contracted providers may request a credentialing application and required documentation checklist by contacting the Credentialing Department.

Recredentialing

To maintain participation in the Leon Health network, providers and facilities are recredentialed in accordance with applicable regulatory and accreditation requirements. The Credentialing Department will notify participating providers and facilities when recredentialing is due and provide instructions for completing the process.
